Is the coupler that goes from the motor to the flex shaft incuded ?
Is the coupler aluminum or black plastic ?
and is it 5mm ?

Coupler is included (it's aluminum- something I whipped up @ work.) It fit my AMMO I had on it, 5mm shaft.

Did you you drill and tap holes for set screws on both ends or did you make a square hole (somehow) on the flex shaft end ?

It's drilled on both ends (5mm on the motor side, whatever size the flex shaft was on the other) and has set screws on both ends. It's all done on a lathe, so everything's concentric. It works very well, and doesn't have any wobble to it at all.
